Vacation Bible School is back this summer and it will be one to remember! VBS will take place from 6-8PM each night from July 22-26. VBS is for children 3 years old - 5th grade. We can't wait to welcome your child to this unforgettable 5-day event!

I hearby give my permission for my child (named above) to attend and participate in Vacation Bible School at Calvary Baptist Church.

I do herewith authorize emergency treatment be given if necessary only after reasonable effort has been made to reach us/me the parent(s) or guardian.

I undersigned do hereby release and agree to hold harmless VBS leadership and Calvary Baptist Church from any and all liabilities or claims for personal injury or illness which may be incurred by my child while attending and participating in VBS activities and events.

I understand and hereby grant Calvary Baptist Church, its staff and leaders permission, that while my child is participating in approved VBS activities, photographs and videotape of my child may be taken for use in promoting Calvary's VBS and its activities. I further release Calvary Baptist Church and its staff and leaership from any claims that I may have against them as a result of my child's picture being used in promoting VBS.
